How does kratom work in the body? 

Kratom, whether green, red or white, primarily contains two main alkaloids: mitragynine and 7-hydroxymitragynine. These substances bind to mu-opioid receptors in the brain, similar to some opioids, but their action differs from traditional opioids. In addition to mu-opioid receptors, kratom also partially affects serotonin and dopamine receptors. This mechanism of action is responsible for the effects typical of kratom: increased energy, concentration, pain relief and increased libido. This interaction is dose-dependent: from stimulation and energy boost at lower doses to calming and pain relief at higher doses. 

After use, kratom is rapidly absorbed into the bloodstream and goes to the liver, where it is metabolized. The average half-life of mitragynine is between 3 and 9 hours, while 7-hydroxymitragynine has a shorter half-life, meaning the body breaks it down faster. Metabolites are then excreted from the body via the urine. 

The duration of kratom in the body depends on several factors:

How long does kratom last? 

As we mentioned earlier, the duration of kratom’s effect depends on several factors. In general, the following applies:

Time of onset of effects

  • When consumed in powder or capsule form – the effects usually occur within 30-60 minutes.
  • When drinking tea – The onset can be faster, often within 20–30 minutes, as the liquid form is absorbed faster.

Duration of effects

  • Lower doses (1–5 g) - Effects such as increased energy, improved mood or concentration usually last 2–4 hours.
  • Higher doses (5–10 g) – Sedative and relaxing effects can last 4–6 hours.
  • At very high doses, it can last up to 8 hours, but with a higher risk of side effects.

How long does kratom remain in the urine? 

The length of time kratom can be detected in the urine depends on several factors, including the dose, frequency of use, metabolism and user’s hydration. In general, the following applies:

Regular use

In regular users who take kratom frequently and at higher doses, its metabolites may occur in the urine for 5–7 days. This is due to the accumulation of alkaloids in the body and the longer period they need to break down.

Occasional use

In occasional users who take kratom as a one-off or only occasionally, detection in the urine is usually possible for 1–2 days.

How long does kratom stay in the blood?

In occasional users, kratom can be detected in the blood for approximately 24 hours after use. With regular use, the alkaloids can remain in the blood for up to 48 hours or more, as they accumulate in the body. Kratom is not part of routine toxicology tests. A specialized test targeting the alkaloids, mitragynine and 7-hydroxymitragynine, is needed to detect kratom in the blood. These tests are expensive and not performed as standard, which means that detection of kratom in the blood is rare.

Factors affecting the time kratom remains in the blood:

  • Dosage – Higher doses mean longer processing times and presence in the blood.

  • Frequency of use – Regular users may have alkaloids in their blood for longer periods of time because their body does not get rid of all metabolites between doses.

Do routine drug tests detect the presence of kratom in the body?

Routine drug tests usually do not detect the presence of kratom in the body. This is because kratom and its main alkaloids (mitragynine and 7-hydroxymitragynine) are not included in standard drug tests. As a rule, they focus on substances such as marijuana, cocaine, opioids, amphetamines and PCP. Kratom alkaloids have a chemical structure different to conventional opioids, although they act on the same receptors. Standard tests therefore do not report a false positive.
